The registration fees include attendance at all conference sessions, conference materials, continental breakfasts, and refreshment breaks. Registration may be completed by mail, fax, or via the Internet at: http://www.epiresearch.org. The early registration deadline is May 30. A confirmation letter will be sent upon receipt of the completed registration form. Payment can be made by check, MasterCard, Visa or American Express. Click here to download a registration form. Payment must be received prior to admittance to the meeting.
**Online registration is now closed. If you wish to register for the meeting, you may do so onsite, in Chicago, beginning Tuesday, June 24 at 4 p.m. Registration for the meeting will be at the Hyatt Regency Chicago, Gold Level.**

If cancellation of your registration is necessary, you may send written notice to SER or call (801) 525-0231. Cancellations made by May 30, 2008 will be subject to a $35 administrative cancellation fee. Confirmed registrants who do not attend the conference or who cancel after May 30th are responsible for the entire registration fee. Substitutions, however, may be made at any time. When calling, be sure to ask for and retain your cancellation number.
